Employee 2015 Annual Contribution Statements Mailed – Active Employees Only
Annual Contribution Statements were mailed to employees on April 25, 2016. The Annual Contribution Statement shows the balance of contributions towards retirement that you have accumulated with the Fund through December 31, 2015. 2015 (1099-R) Tax Forms Mailed
All 1099-R tax forms were delivered to the US Post Office on January 26, 2016. If you do not receive your 1099-R by February 12, 2016, please contact our office for a reprinted form. Note: If you moved or have a new address, you must sign and complete a Tax Center Change of Address Form as soon as possible. Please download the form and mail or fax the form to the Fund office.
