Employee 2015 Annual Contribution Statements Mailed – Active Employees Only

Annual Contribution Statements were mailed to employees on April 25, 2016. The Annual Contribution Statement shows the balance of contributions towards retirement that you have accumulated with the Fund through December 31, 2015. This statement is intended to be informational only. Please review the statement carefully and contact the Fund if you have questions. Click ‘Download Document” for additional information.

New Health Benefit Tax Form (1095-B) Mailed On February 26

This year the Fund is supplying annuitants covered under the health plan with a new tax form called a 1095-B. If you are an annuitant that received health coverage from the Fund in 2015, please click ‘Read More’ for additional information.

2015 (1099-R) Tax Forms Mailed

All 1099-R tax forms were delivered to the US Post Office on January 26, 2016. If you do not receive your 1099-R by February 12, 2016, please contact our office for a reprinted form. Note: If you moved or have a new address, you must sign and complete a Tax Center Change of Address Form as soon as possible. Please download the form and mail or fax the form to the Fund office.
