Support Resources

Whether you want to maintain your health, are looking for some health information, or are trying to manage a health condition, you will have access to personal services and clinical support that may meet your unique healthcare needs. Below are just a few of the programs available to UnitedHealthcare members.

Need Help Getting Started?

A personal health assessment is available when you register or log in to myuhc.com, with immediate results that help you to focus on your health risks and recommendations on programs you can benefit from.

Work With a Wellness Coach

Your plan includes a wellness coaching program. You will be able to meet one-on-one, by phone and email, with a certified wellness coach. The coach will work with you to help motivate you and to create a personalized action program. Coaching programs include:

  • Weight management
  • Exercise
  • Nutrition
  • Tobacco cessation
  • Stress management
  • Heart health
  • Diabetes

Care Management

UnitedHealthcare members who are experiencing emotional and physical health conditions will find a variety of resources and people to talk with for help in managing their health. These confidential services are available at no extra cost, and were created to help you live a healthier, happier life — we encourage you to take advantage of them.

If you have diabetes

The Diabetes Management Program will help you manage the “ABCs” of diabetes:

  • A1C (blood sugar control)
  • Blood pressure, and
  • Cholesterol

The program connects you with nurses and advisors who can help you control your diabetes, manage your medications, and improve your diet and exercise. Call (855) 318-7347 and ask to speak with a Wellness Coach. You will receive a personalized one-on-one consultation to get you started on your path to a healthier life.

If you have kidney disease

Kidney Resource Services can help if you are on dialysis or about to start dialysis. The program connects you with nurses who can help you understand your condition. Our nurses will also help you find preferred dialysis centers to help make certain you’re getting the right care. Still not sure where to start? If you have coronary artery disease

The Coronary Artery Disease (CAD) Program can help reduce unnecessary hospitalizations and healthcare costs, and improve your quality of life. Call (855) 318-7347 and ask to speak with a Wellness Coach. You will receive a personalized one-on-one consultation to get you started on your path to a healthier life. The program may help you:

  • Reduce your risks and maintain a healthier lifestyle
  • Follow your doctor’s treatment plans and medication recommendations
  • Get the needed tests and procedures to help you manage your condition
  • Manage other health issues
