The Cook County Pension Fund (CCPF) has arranged for Guardian, one of the largest dental providers in the country, to offer voluntary dental insurance to CCPF annuitants and their family members at competitive rates.

IMPORTANT: CCPF does not subsidize or administer the dental plan. If you elect coverage under the Guardian Dental plan, the premiums will not be deducted from your annuity. Premiums must be paid monthly by check or money order and mailed to the address below, or through an automatic debit to a credit/debit card with a Visa or MasterCard logo.
