Voluntary Dental Benefis
The Fund has partnered with Guardian Dental to offer a voluntary Dental Program to our annuitants and their eligible dependents.
Costs
All costs are to be paid directly to Guardian. The Fund will not handle any payment to Guardian for the cost of the dental program. If you choose to participate in this program, you will be solely responsible for all costs associated with the benefit. The Fund does not subsidize this program.
HMO
Single: $11.91 per month
Family: $31.46 per month
PPO
Single: $35.29 per month
Family: $67.28 per month

For assistance in locating dentists or to determine if your dentist is in the Guardian Dental network, visit the Guardian website or call (877)522-2524. **Please note Cook County Networks listed are for Active employees only and do not apply to the Fund's Annuitants. For PPO choose "First Commonwealth" and for HMO choose "First Commonwealth DHMO"

New annuitants can read the information below to enroll. In addition there is Open Enrollment each November during the Fund's Health Benefit Open Enrollment
HOW TO ENROLL
For questions about the dental benefits offered and for assistance with the enrollment decision, please call (877)522-2524 or visit a Special Dental Enrollment Session hosted by Guardian in April.
There are three ways to enroll:
