All 1099R tax forms have been mailed.  If you do not receive your 1099R by February 7, please contact our office for a reprint.  Note: If you have moved, you must supply the Fund with a change of address prior to any reissue of a 1099R.

The amount in Box 5 on your 1099R form represents the employee contribution or non-taxable amount you received in 2013.  It is the difference between your gross amount (Box 1) and the taxable amount (Box 2a). The amount in Box 5 is NOT related to any health insurance deduction.
