The Cook County Health and Hospitals System recently notified the Cook County Pension Fund (CCPF) that effective August 1, 2018, the County doctor will no longer complete the County Physician Statement (CPS) for Cook County and Forest Preserve District employees applying for ordinary disability.
Until further notice, ordinary disability applicants are advised to send originals of their completed application, federal income tax form (DIS0006),and Attending Physician Statement (APS), as well as a copy of their driver’s license or state I.D. to the Fund. Only original documentation will be accepted. CCPF apologizes for this inconvenience.
