New Health Benefit Tax Form (1095-B) Mailed On February 26
Please note: 1095-B forms were were mailed on February 26. If you do not receive the form by March 14, please contact the Fund for a reprint.
About the 1095-B Form
This year the Fund is supplying annuitants covered under our health plan with a new form called a 1095-B.
If you are an annuitant that received health coverage from the Fund in 2015 you will receive a 1095-B form from the Fund via mail. Forms were mailed on February 26.
The 1095-B form is for information purposes only and is not required to complete your tax return. The 1095-B will only indicate that you had health coverage for a specified period during 2015.
